Employee Qualifications

 

Must be committed to the care and support of those we serve, have a nurturing spirit, and be willing to share your life with others:

 

  • 18 years of age or older

  • High School Diploma or GED

  • Valid driver’s license with a good driving record for at least 3 years

  • Ability to pass a basic math and reading test

  • Our Family Living Director positions require that applicants be 21 years of age or older and have direct care and supervisory experience.

Direct Support Professional (DSP)

As a frontline employee, the DSP will provide and assist with daily living activities, including preparing meals and assisting with personal hygiene.  Develop a rapport with residents and cultivate a safe and supportive relationship.  Update person’s progress notes on a daily basis and provides supervision and guidance for persons during skills building activities. The person must be friendly, outgoing and will to the person’s participation in the community, social and leisure activities.

 

Requirements:  Must be able to pass a criminal background check, CPR, First Aid, TME, CMT, Physical, High School Diploma/GED,

 

Lead Direct Support Professional

The Lead Direct Support Professional is responsible for assisting the House Manager and other program staff in providing on-site management and supervision of the facility and to be in compliance with health, safety, funding and program standards of the organization's policies and procedures.  Assists the House Manager with the delivery of support and management of resource for the people we serve. This position may also be responsible for assisting the person(s) with scheduling of activities, and arranging  Direct Support Professionals.

 

Requirements:  Must be able to pass a criminal background check, Physical,  High School Diploma/GED 2+ years of work related exp., First Aid, CPR, TME, CMT


Licensed Practical Nurse Case Manager (LPN)

Under the direct supervision of a Registered Nurse (RN) LPN will provide services acting in the role of a Case Manager including such services as:  training, testing and occasional supervision of unlicensed personnel who are responsible for the day to day direct care of developmentally disabled persons, review of patient records, interpretation of medical orders, assessment of medical needs and planning for the care of such person.

 

Requirements:  Physical,  LPN MD and DC License, CPR certification, experience in DD field is preferred.

 

Registered Nurse (RN)

As an RN who have specialized formal, post-basic education and who function in highly autonomous and specialized roles.They assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records.   Supervise LPN and home staff who are responsible for the day to day direct care of developmentally disabled persons. May advise LPN and others on health maintenance and disease prevention or provide case management.

Consult with physicians, staff, and Persons on health-related issues as needed or as requested, consult and train staff regularly on DDS documentation requirements for content and format.  Review and sign off on RN monthly monitoring and running notes.

 

Requirements:  Physical,  RN MD and DC License, CPR certification, 2+year experience in the DD field
